Issuing National Identity Cards
- Application for a new Identity Card (New Identity Card - DPR 1, Obtaining a duplicate - DPR 7, Amending the NIC - DPR 8)
- A person aged 15 years is subjected to registration of the Identity Card.
- Original Birth Certificate or Probable Age Certificate and a photocopy for the applicants less than 50
- For over age 50, If there isn’t the original birth certificate or probable age certificate, there should be document search result chart and optional documents (certified certificate of school leaving, bridging certificate, Birth certificate of a child that applicant’s birth date is mentioned, birth certificate note, estate leaving certificate, certified copies of the passport , Registration card issued by the estate superintendent, birth certificate)
- Citizenship certificates if born abroad
- Iconic standard four passport size colour photographs
- If you are applying for an NIC for the first time, the certified photocopy of the National Identity Card of the applicant's parent / guardian which is certified by Grama Niladhari
- Marriage certificate to get husband's name
- Certificate of Samanera or an upasampada certificate for Buddhist monks
- Disrobed certificate issued by the Department of Buddhist Affairs, if the Buddhist monk is disrobed.
- Certificate of Service obtained within 3 months if required
- For missing identity cards, the original copy of the complaint made by the police
- Identity card currently being used for amendment
- Formal affidavit with stamp (if needed)
- Revision of fees for obtaining an Identity Card from 01.09.2018
Obtaining the Identity Card for the first time Rs. 100.00
Obtaining the amendment identity card Rs. 250.00
Obtaining duplicates for missing identity cards Rs. 500.00
- A first-time applicant over the age of 15 will be subjected to fine of Rs. 250.00 in addition to the above Rs.100.00
Including a name to birth certificates / Changing a name in the birth certificate
For those over 7 years of age -
- The Christian Certificate, Student Performance Index
- Newspaper advertisement, Bank Book
- National / Posture Identity Card, Marriage Certificate
- Report from the government or other institution
- Certified copy of voting register
- Birth certificates of children
- Certificates of education
- Driving license
- Passport
- Other relevant documents that the suggested name is mentioned
- You must submit at least three of the above documents
In addition to that,
- Relevant application
- Original birth certificate to be amended
- Letter of Grama Niladhari with the signature of relevent Divisional Secretary which certifies that same person is identified by two names.
- The affidavit which certifies the same person bears the name mentioned in the original birth certificate and the suggested name to be i:ncluded to the birth certificate.
NB ;
- If the person whose name to be changed is below 21 years of age, the relevant statement and the affidavit should be forwarded by the parent / guardian.
- The new name should have been used at least one year.
Children below the age of 7 years
- Original birth certificate to be amended
- Parent's marriage Certificate
- Mother's / father's National Identity Card
- Affidavit that ensures there is no objection of parents
- This declaration should be handed over by the child's parent / guardian
Rs. 50.00 should be paid when this declaration is handed over.
Correction of a mistake that occurred in the mother’s column in the birth certificate
- Original birth certificate to be corrected
- Original birth certificate of the mother
- Copy of the national identity card of the mother
- Original marriage certificate of the parents
- Rs. 50.00 should be paid
Changing the 4th, 6th, 7th, 8th columns (father’s column, grandfather’s column and including the civil status) or including new details to those columns and changing the details of 5th column
- Original birth certificate to be corrected
- Original birth certificate of the father /Original birth certificate of the mother
- Copy of the national identity card of both father and mother
- Original marriage certificate of the parents
- Rs. 50.00 should be paid
Registration of past births
- Hospital birth description (Original copy)
- Photo copy of vaccine card
- If the parents are married, original copy of the marriage certificate e
- Original Birth Certificates of Parents
- Photocopies of National Identity Cards of both mother and father
- Rs. 50.00 should be paid
Please come to the district registrar division with the above documents and please be kind enough to come mother, father and the child too.
Registration of past deaths
- Original hospital death description if died in a hospital (Original copy)
- Death report issued by Grama Niladhari if died at home
- Condolences/ death notices
- Receipt that paid for the coffin
- The letter of monk or priest who has done the religious rites at the funeral
- A letter from the relevant funeral aid society
- A blood relation of a dead person or a person who was with the dead person when death happens or a person who was taking care the dead person should be presented for the registration of the death
- Rs. 50.00 should be paid
